Reema Health Moves from Disconnected Tools to a Seamless, All-in-One Care Platform
Reema Health, a U.S.-based home healthcare company, needed a mobile-first care platform to support field teams with secure communication, centralized patient information, and smoother task management. What was the main challenge in this project?
Reema Health relied on disconnected tools that created fragmented communication, limited patient visibility, slower workflows, and growing data security concerns. Their care teams needed a unified digital system to manage communication, patient profiles, member information, and field operations more efficiently.
What was your solution or approach?
We designed and developed a mobile-first platform built around four core modules: Authentication, Feed, Profile, and Members. The Feed enabled real-time communication, Profile and Members centralized patient and care-related data, and Auth0 integration strengthened secure access. The platform was designed with a care-centric UI/UX approach to keep workflows simple, structured, and scalable.
What was the outcome or impact for the client?
The new platform helped Reema Health move from fragmented tools to a seamless care management experience. Care teams gained better visibility, faster access to patient information, improved collaboration, and more secure communication, reducing the risk of patients falling through the cracks.
